Hello, everyone. For our global community project I chose South Tarawa, the capital of Kiribati, a Pacific nation of low-lying coral atolls spread across an area of ocean about the size of the continental United States. About 136,000 people live in Kiribati, and roughly two thirds live in urban areas, most of them crowded onto the narrow strips of land that make up South Tarawa (World Bank, 2026).
I chose this community because I care for many patients with diabetes and tuberculosis in Phoenix, and Kiribati faces both at a scale I had never imagined. World Bank data show diabetes prevalence of about 24.6% among adults aged 20 to 79 and tuberculosis incidence of about 945 per 100,000 people in 2024 (World Bank, 2026). A newborn could expect to live 66.6 years, and about 39 of every 1,000 infants died before age one.
Our readings describe social determinants of health as the conditions where people are born, live, work and age (Stanhope & Lancaster, 2020). In South Tarawa, three stand out. First, the physical environment: roughly one person in seven has safely managed drinking water, and one in four has safely managed sanitation, which matters on a crowded atoll where groundwater sits close to the surface. Second, crowding: population density is high, and crowded housing helps tuberculosis spread. Third, the food environment: imported processed foods have replaced much of the traditional diet of fish, coconut and breadfruit, which researchers link to rising obesity and diabetes. Climate change ties these together; a modeling study projected that wave-driven flooding could make most atoll islands uninhabitable by the mid-21st century by damaging infrastructure and freshwater supplies (Storlazzi et al., 2018).
On cultural competence, I want to be honest about my lens. I am an American nurse who has never been to Kiribati, and I could easily describe the community only by its problems. I-Kiribati culture has strong family networks, community meeting houses called maneaba and deep knowledge of living with the sea. A culturally competent assessment will look for those strengths as carefully as for risks, and I will rely on sources written with or by Pacific communities wherever I can.

Lucia, your choice of a peri-urban community outside Lima interested me because, like South Tarawa, it is defined by rapid migration into crowded settlements with uncertain water. I wonder whether you found that water access shapes health there through infectious disease, the way it does in Kiribati, or more through cost and time spent collecting it.
