Team Data Rangers: A Charter and Work Plan for Tracking a Healthy People 2030 Objective
Team Purpose
We will produce an accurate, evidence-based judgment about whether one Healthy People 2030 objective is on track, and we will learn to work as a team that could do the same at our own organizations.
Communication
Primary channel: a group chat for quick questions; a shared folder for all drafts; a weekly 40-minute video meeting on Wednesdays at 6:30 p.m. Arizona time. Response time: 24 hours on weekdays, 48 hours on weekends. Decisions are recorded in a decision log at the top of the shared folder.
Accountability
Each task has one owner and a due date in the work plan. If an owner will miss a date, they tell the team at least 48 hours ahead and propose a fix. Two missed dates without notice trigger a team conversation, then a note to faculty if the pattern continues.
Psychological Safety Agreement
We agree that any member can say "I don't understand this dataset" or "I made an error" without blame. Research on teams links this kind of climate to learning behavior and performance (Edmondson, 1999). The facilitator will ask at the end of each meeting whether anyone has a concern they have not raised.
Conflict Ladder
1. Talk directly with the person within two days.
2. Bring it to the facilitator if unresolved.
3. Discuss at the next meeting for no more than 15 minutes.
4. Ask faculty for guidance.
Teamwork Practices
Teamwork research identifies core components such as team leadership, mutual performance monitoring, backup behavior, adaptability and team orientation, supported by shared mental models and closed-loop communication (Salas et al., 2005). We will build in backup behavior by pairing members on each data source, and closed-loop communication by confirming each assignment in writing.
Healthy People 2030 Work Plan
Objective chosen: NWS-03, lowering adult obesity (Office of Disease Prevention and Health Promotion, n.d.).
| Role | Member | Task | Due |
|---|---|---|---|
| Data lead, NHANES | Public health nurse | National prevalence and trend | Week 5 |
| Data lead, BRFSS | Charge nurse | State patterns and Arizona | Week 5 |
| Disparities analyst | Nurse practitioner | Differences by sex and age | Week 6 |
| Synthesis and slides | Nurse educator | Draft deck and timing | Week 7 |
| All | All | Rehearsal and final decision on progress | Week 8 |
Review
We will revise this charter after the presentation, as the second charter assignment requires.

What the DNP 715 Module 2 instructions ask for
The posted syllabus grades two Team Charter Assignments for 50 points in total. In the first, teams develop a charter that sets expectations for communication, accountability, psychological safety, conflict management and teamwork, and begin planning the Healthy People 2030 Progress Assessment by identifying roles, timelines and responsibilities. The second charter, later in the term, revises this one after the presentation and plans the final workflow redesign project. Expect to submit one charter per team. Choose your Healthy People 2030 objective early, because the data sources you will need depend on it. Canvas may provide a template. Agree on the charter in a meeting rather than by email, so every member has a chance to raise concerns before the rules are set.
How this DNP 715 Module 2 example is built
The charter is organized around the five expectations the syllabus names, communication, accountability, psychological safety, conflict and teamwork, each with concrete rules. Times, channels and thresholds are specific enough to enforce. Two sources support the safety agreement and the teamwork practices, and each is turned into a behavior the team will actually do. The work plan table names the objective, roles, owners and dates for the Healthy People 2030 assessment, which is the second half of the assignment. A closing line commits to the revision required later in the course. The work plan table assigns a data source to each member, which prevents the common problem of two people analyzing the same survey while another is ignored.
Where the marks sit in the DNP 715 Module 2 rubric
Team charters are usually graded on whether every required element is addressed, how specific and realistic the expectations are, attention to psychological safety and conflict, a clear work plan with roles and timelines, use of teamwork evidence, and evidence that the whole team contributed. Faculty look for charters that would help a team when something goes wrong. Turning research into practices, such as pairing members for backup, shows application. A work plan with owners and dates shows readiness for the project. Charters that plan for their own revision show that the team expects to learn. Charters that state what happens when a rule is broken, not only what the rule is, give faculty confidence that the team can manage itself.
DNP 715 Module 2 help with common mistakes
Charters often promise respect and communication without saying how. Write rules with times and numbers. Plans also go wrong when nobody owns a data source; list the objective, each source and its owner. Agree on file names and folder structure. Discuss how you will handle a member who goes quiet. For help with your charter and Healthy People 2030 work plan, share the template and your meeting notes with the desk. Check the Healthy People 2030 website for your objective's baseline, target and data source before assigning tasks. Keep the charter short enough that members will actually reread it before the second assignment.
